Prior to your appoitment, you will receive forms to fill out online, cutting down on paper handling in the office at the time of your appointment.
Please call the office from your car when you arrive for your appointment. The front desk will re-verify our screening questions and let you know when we are ready for you to come in.
Guests are allowed for minor children, those who need interpreting, or those who have a medical need. All other guests are asked to remain at home or in their cars.
Please remember to wear your mask from home when entering the office. Masks must be worn until treatment is ready to begin.
A hand sanitizing station will be available at the office entrance for your convenience.
Our team will be taking your temperature at the front desk. Any individual with a temperature greater than 100.00 will need to reschedule.
